Your Privacy
First Avenue Family Practice values the privacy of all our patients. No patient information is given to a third party without your consent.
Our staff are trained in patient confidentiality and have signed a “confidentiality of information” contract.
Your patient records are fully computerised and are only accessible by the doctors and nurses.
My Health Record
Our doctors can help you to create a secure medical summary and decide what information you would like to upload to your My Health Record. We encourage you to make a visit to do this to ensure that information is correct, up to date and what you would like to have there.

Complaints
Please talk to your GP or the practice manager, Adele Di Manno if you have any complaints or suggestions. Alternatively you may use the feedback boxes which are located in the waiting rooms. If you have a complaint about your care that is not resolved in this way, you may contact the Health Care Complaints Commission.
